Employee's foot lacerated when entangled in chain drive

At approximately 4:20 p.m. on December 21, 1994, Employee #1 was attempting to change the speed control of a conveyor that removes dirt, leaves, and other debris from fruit that the company processes and packages for shipment. To change the speed, the conveyor must be in operation. A ladder and walkway are normally located at the side of the machine for employees to use while the machine is in operation. The ladder and walkway, which are not attached to the machine, had been removed by other employees to change a bin placed under the conveyor to catch debris. Employee #1 decided to change the speed of the conveyor by climbing up the side of the machine to reach the control. He was on his way down when his left foot slipped between an angle iron and a brace. His foot became entangled in the chain drive of the machine, and was lacerated.
