Investigation Summary
On May 28, 1996, Employee #1, of Polycarbon, Inc., a manufacturer of carbon fiber and graphite products, was operating a Seydell stretch-breaker machine in the spinning room. This machine processed a ribbon of carbon fiber material by moving it under tension through a series of rollers. While he was operating the machine, Employee #1 noticed a flaw in the material moving through the machine, and he touched the carbon fiber ribbon with his fingers to see if the material was good. As he did so, his fingers became caught in the fibers and were pulled into the machine's rollers. They eventually became wedged between one of the rollers and a nip point guard in front of the roller. A coworker used a pry bar to wedge back the guard and free Employee #1's hand. He sustained three broken fingers on his left hand and subsequently underwent surgery to repair the damage. The accident was caused by an inadequate guard, which allowed the employee's fingers to enter the danger zone around the machine's rollers and, ultimately, to become wedged between the guard and one of the rollers.
