Employee's finger cut by saw blade, later amputated

On June 14, 1995, Employee #1, of Anthony Manufacturing Co., was in the crating department cutting wood in order to assemble shipping crates for refrigerator doors manufactured by the employer. He attempted to remove a piece of scrap wood that was obstructing the local exhaust ventilation system on the Rockwell radial arm saw, serial #W6266, that he was operating. Employee #1 went around to the back of the saw, disconnected the exhaust duct, and stuck his hand into the opening to remove the scrap wood. The saw was turned off, but the blade was still moving. When Employee #1 grabbed the piece of scrap wood, his right index finger struck the blade and was severely lacerated. Later part of his finger later had to be amputated. The accident was caused by a failure to stop and block the saw blade prior to reaching into the saw to remove the scrap lumber obstruction.
