Employee killed in fall down pile hole

Employee #1 was on a job site inspecting for city building requirements. The pile holes required inspection prior to the placement of reinforcement steel and concrete to ensure that there was not an undue amount of cave-in material in the bottom of the holes. According to a witness, Employee #1 lifted a cover from one of the holes and walked into it. He sustained multiple fractures as a result of the fall and was killed. Employee #1 may have been distracted by some other activity on the construction site.
