Investigation Summary
At about 2:15 a.m. on October 3, 1994, Employee #1, of the Anheuser-Busch packaging and shipping department, was checking the alignment of flat metal conveyor links of a bottle conveyor to determine why so many bottles were falling. With his hand on the moving conveyor at an area where the belt loops under to return, he leaned over to look at the underside of the system. His hand drifted to this end area and a finger became caught between the belt and a metal bar bridge located next to a bottle catch basket, a gap slightly less than 1/2 inch. Suffering a fractured finger, Employee #1 was hospitalized.
