Investigation Summary
At 2:35 p.m. on August 1, 1994, an employee was replacing one of three knives in a trimmer. The employee removed knife and placed it in a protective guard. He then proceeded to place the knife on a work table 4 ft away. While moving to the table, the employee tripped over a vacuum duct that was 8.5 in. high. The tripping caused the knife to dislodge from the protective guard, cutting the employee's right hand palm. He suffered severe cut to his right hand, and was hospitalized.
