Investigation Summary
Employee #1 was removing the nuts holding concrete sound barriers along the freeway. The nuts were to be removed in sequence, and last two were not to be removed until crane cables were attached to lifting lugs. Apparently, Employee #1 got ahead of the workman attaching the cables and the panel came down, crushing the deceased.
