Investigation Summary
On July 28, 1994, Employee #1, of Fireball Electric Inc., was working inside a canopy that was above the gas pumps of an AM/PM mini-market being constructed in Santa Clarita, CA. His job assignment was to pull communication cable from the electrical room up through a column to a J-box inside the canopy to connect an ATM. The canopy is constructed of sheet metal, making it very hot inside. After 45 minutes of pulling the cable with a pull string, Employee #1 began suffering from shortness of breath, chest pain, and excessive sweating and lost consciousness. His coworkers discovered him and cooled him down with ice water. Employee #1 was treated by paramedics and was hospitalized for three days, recovering from heat exhaustion. No accident-related violations were found because of both the inability to assess what the temperature and ventilation conditions were at the time of the accident and the lack of a standard.
