Employee injured when caught in belt drive

On July 13, 1995, Employee #1, of Recycle Base Materials, Inc., Sun Valley, CA, entered a 2 foot wide, 4 foot tall opening underneath a universal saw crusher to grease the bearings on the push drum and lower conveyor belt. He slipped and became caught in the running belts. His arms and upper body were dragged into the system. The operator foreman felt the engine slow down. He shut the system off and found Employee #1 inside. He suffered multiple severe injuries to the upper body and head. Employee #1 was still in the hospital as of December 12, 1995.
