Employee injured in fall with aerial device

At approximately 3:30 p.m. on June 29, 1995, Employee #1 and a coworker were servicing a sign. Employee #1 was standing in a personnel basket that was bolted to the end of the boom of a truck-mounted crane. The coworker was operating the boom from the truck bed, and had extended the boom about 20 ft for Employee #1 to reach the sign. Employee #1 removed the sign's plastic cover, cleaned some bulbs, and had begun painting the sign when the boom suddenly fell. Employee #1 was not using a safety belt and lanyard and landed on the ground next to the basket, sustaining two broken legs, damage to the left side of his head and ear requiring stitches, and the loss of five teeth. He was hospitalized for five weeks, had surgery on both legs initially, and additional surgery later on the left leg.
