Investigation Summary

Investigation Nr: 170151302
Event: 06/01/1995
Employee's hand crushed by falling anode

Employee #1 and a coworker, of Hawker Pacific, were preparing to electroplate a part for a 727 aircraft. To do this, they needed to place an approximately 150 lb anode inside the part. Employee #1 was grinding the anode into the opening of the part, which was suspended beside his head on an overhead crane with a copper hook. The coworker was at the crane's controls. The anode was half way into the part when the coworker began to lower it step by step. Employee #1 was guiding the post when the hook failed, dropping the anode into the part. Employee #1's hands were crushed between the part and the anode. He suffered broken bones and other injuries to both hands.
