Investigation Summary
At approximately 1:00 p.m. on January 5, 1995, Employee #1, a foreman on a reinforcing steel crew for Vista Steel Co., Ventura, CA, and coworkers had set up reinforcing steel for a 25 foot high concrete tank and tied it to the ground. The horizontal steel spacing was about 9 in. Employee #1 was on a scaffold at the top and had started to descend the steel when his hands slipped and he fell approximately 20 1/2 ft. He landed on an employee of M.A. Mortenson. Employee #1 suffered no serious injuries and was released from the hospital in three hours. The employee was wearing gloves but they were wet.
