Employee injured in fall from rear of truck van

At 9:30 a.m. on February 11, 1993, Employee #1, of Goodwill Industries, was unloading a sofa from the back of a truck van. While stepping backward out of the van, he fell 4 1/2 feet to the ground. Employee #1 sustained a head injury. He was taken to Simi Valley Adventist Hospital, and was released after 24 hours of observation.
