Employee injured in fall through roof opening

Employee #1 was working on the 35 ft long by 65 ft wide flat roof of a commercial building under construction. The roof had a 4 ft long by 4 ft wide access hole cut in it which Employee #1 had been instructed to finish. He placed a 2 in. high by 12 in. wide by 4 ft long section of joist in the hole, over which a 1 in. high by 4 ft long by 4 ft wide plywood would be placed. As Employee #1 leaned over to nail the joist section into place, he lost his balance and fell approximately 17 ft through the hole onto cement. He sustained cuts to his arm as he fell, and a fractured elbow when hitting the ground.
