Employee burned in flash fire

Employee #1 was supervising an oil field pipe flange welding repair when the material in the pipe, which was improperly blended wastewater (water and oil), came in contact with hot water. The mixture exploded, causing flaming gas and hot water to shoot out of a vent pipe at head height, striking Employee #1, who was 30 feet away. His shirt caught on fire and he sustained burns on multiple parts of his body. Employee #1 was hospitalized.
