Investigation Summary
A maintenance department welder, who was working in a fabrication shop/warehouse of a solar power generating station, was mounting a wheel on the left rear axle hub of a barbecue trailer. (This task was not one of his normal job duties, and there was no supervisor in the shop at the time.) The trailer was supported by a jack, but the vehicle was not effectively chocked to prevent movement. When the employee placed the wheel on the hub, the trailer tipped over and fell onto his left leg. The trailer pinned him to the concrete floor and shattered the bones in his lower left leg. The employee was hospitalized for his injuries.
