Employee injured in same-level fall on slippery floor

At approximately 2:00 p.m. on May 10, 1996, Employee #1 was removing the peel and seeds from cantaloupes. She started walking to another location when she slipped on some seeds on the painted concrete floor. Employee #1 suffered back, neck, and head injuries and was transported to the hospital, where she stayed for approximately 4 to 5 hours. The floor was also wet from water in the area. The employer was cited for not maintaining a clean and clear floor.
