Investigation Summary
At approximately 2:30 p.m. on December 28, 1995, Employee #1 was supervising inmates as they installed a sheet metal vent duct for a pot washer. He climbed the 6 ft stepladder to check on the work. When Employee #1 put his foot on top of the 6 ft high pot washer, the ladder went over sideways. He fell, striking his head on the pot washer machine on the way down. Employee #1 suffered head injuries and was transported to a hospital, where he stayed for one week. He had not been facing the ladder at the time of the accident.
