Investigation Summary
At about 11:30 a.m. on May 18, 1993, Employee #1 was assigned to repair a noisy belt on a motor home. He got a can of WD-40 to lubricate the belts but needed to run the belts in order to locate the one making the noise. Employee #1 asked a coworker to help him by starting the motor while he was listening to the belt from under the motor home. Employee #1 got under the motor home and was feeling the belts with his left hand when the coworker mistakenly started the motor. Employee #1's left hand was caught in the belt and pulley. His thumb (to the 1st joint) and his index finger (to the 2nd joint) were amputated. Employee #1 and the coworker could not see each other. This lubrication could have been done by one person with a remote start.
