Employee breaks wrist in fall from stepladder

At about 10:00 a.m. on May 3, 1993, Employee #1 went to the stock room to get a pet taxi for a customer. The box of pet taxis was on a pallet on a top rack. Employee #1 got a 6 foot metal stepladder, climbed to the fourth step, and began to pull out the box. The box, which weighed 44 pounds, was too heavy, so Employee #1 asked a coworker for help. While Employee #1 was handing the box down to his coworker, he could not hold it any longer and dropped it. The box fell against the bottom part of the ladder's legs, and Employee #1 lost his balance and fell headfirst toward the concrete floor. He put his hand in front of him and broke his right wrist. No ladder training had been given.
