Investigation Summary
At approximately 2:00 p.m. on January 15, 1993, Employee #1 was demolishing an old metal waste hopper structure with two coworkers. They had been working on this project for several days and had most of the structure down. Employee #1 was using a torch to cut the metal. He had cut one side of a horizontal beam about 8 ft long that had been held by two columns. At this time, the beam was only attached at one column. Employee #1 went down one tier (3 feet) to cut off a section at the columns when the iron beam above him fell and struck him on the head. His head was cut but he was not hospitalized. The weld on the other column was rusted. No hard hats were being worn. No job training had been provided. Employee #1 was a fork lift operator. No plan had been established to demolish the structure; the upper tier had not been dismantled before starting on the lower tier.
