Investigation Summary
At approximately 8:15 a.m. on November 16, 1992, Employee #1, of Hacienda Harvesting, Inc., was picking lettuce, working behind a machine that had a conveyor unit. The ground was wet and littered with a lot of lettuce leaves. Employee #1 slipped on a lettuce leaf as she bent down to pick a head of lettuce. She fell against a step on the machine 1 1/2 ft above the ground, striking her head. Employee #1 was taken to the hospital and released. No citation was issued.
