Investigation Summary
On July 15, 1993, Employee #1 came to work at 4:15 a.m. She and the other employees were to get the restaurant ready to open at 5:30 a.m. The maintenance crew, who had been steam cleaning, was not finished and lots of water remained on the floor. As Employee #1 walked through an area where there was lots of water, she slipped and fell, hitting her head and back. Employee #1 was taken to the hospital but did not stay. Steam cleaning should be done before the rest of the employees come to work to prevent this kind of accident.
