Investigation Summary
At approximately 1:00 p.m. on March 1, 1995, Employee #1, of Crest Paper Box Co., was cleaning the roller of a Miehle offset printing press, model #29. He jogged the press with his left hand on a jogging button and cleaned the press with a rag in his right hand. The cleaning rag became caught in the rollers and pulled Employee #1's right hand into the roller. Employee #1's hand was crushed and sustained multiple fractures.
