Investigation Summary
Employee #1 and a coworker, of Paramount Pictures, were repairing the HVAC unit on the roof of the Crosby Building. Employee #1 was climbing a ladder to access the roof when he lost his grip and fell head first 12 feet to a catwalk, and then another 8 feet to a ceiling. He was hospitalized with multiple facial cuts, a fractured nose, and a fractured right wrist and shoulder.
