Investigation Summary

Investigation Nr: 170607006
Event: 05/06/1994
EMPLOYEE FRACTURES ARM AND BRUISE CHEST- ALIGN BOLT HOLES

Employee #1 was assisting two other pipefitting employees in handling a 6-foot-diameter flange. The flange weighed 2,507 pounds. They were attempting to align bolt holes so this blind flange would be secured in place. However, they were unable to get the lifting apparatus into proper location (crane or other wise), due to the piping in and around this connection. The Flange moved, and the movement caused the flange to pin the employees against an adjacent wall. As a result, Employee #1 fractured his left forearm and bruised his chest.
