Investigation Summary
Employee #1 was operating a forklift in a warehouse when the blades disengaged a horizontal beam on a rack. He climbed the mast of the forklift to attempt to resecure the beam, and was working from the elevated forklift blades when he slipped and fell 10 to 12 feet to the concrete floor. The product toppled onto him, fracturing his head and injuring his neck, chest, and left wrist.
