Employee struck in eye by flying grinder disc piece

Employee #1 was working on a steam generator boiler retube project at the Southern California Edison Plant. He was grinding metal pipes (2 in. OD, 1 1/4 in. chrome steel tubing, carbon steel, 0.18 in. wall thickness) in half with a peanut grinder (Astro Pneumatic AP210-4 in. disc). He was wearing a 0.040 in. face shield, but no safety glasses. The disc disintegrated and a piece penetrated the faceshield, causing serious injury to his left eye and cheek. A serious accident-related citation was issued.
