Employee eye injury from fireproofing gun

At 1:30 p.m. on January 14, 1994, Employee #1 was struck in the head, face and eyes when the fire proofing gun and hose assembly he was using ruptured. Employee #1 was atop a 20-foot-high scissors lift spraying fire proofing inside a building. He was not wearing face or eye protection. A swivel, inside the hose assembly, had been replaced by a swivel from the local machine shop rather than from the spray gun manufacturer. The machine shop swivel failed under a pressure of 800 to 1,000 pounds per square inch. Employee #1 was hit in the face, eyes and head by the fire proofing material. Employee #1 was rendered unconscious and sustained extensive eye damage requiring hospitalization.
