Investigation Summary
At about 2:30 p.m. on January 12, 1994, Employee #1 fell from a platform that was elevated by a forklift. He sustained facial fractures and a fractured wrist. Employee #1 was transported to Memorial Hospital. Investigation indicated that the platform was not secured to the mast or forks of the forklift; the employees were not trained on how to lift employees safely; the platform was not guarded next to the mast; the platform was not provided with guardrails and toeboards on all sides; and the operator did not remain in the operating position while Employee #1 was elevated.
