Employee injured in fall, unsecured lanyard

At about 10:30 a.m. on January 10, 1994, Employee #1 was installing splice whalers on wall forms on the exterior of a new structure. He was standing on a whaler about nine feet above the work platform that was 13 feet above a concrete deck. He was wearing a safety belt with lanyard. The lanyard was not secured to a substantial member of the structure and of a securely rigged line. Employee #1 fell and hit the top rail of the work platform, he went over that rail and fell to the concrete deck sustaining fractures requiring hospitalization.
