Investigation Summary
At 6:00 p.m. on November 3, 1993, Employees #1 through #9 were at stage #6 at the Warner Studios in Hollywood, filming the movie "Blown Away." A tear gas canister, which was provided by the L.A.P.D bomb squad, was placed on a set as part of the set dressing. The canister was accidentally blown by itself and discharged. Eight employees were treated for eye and skin irritation at local hospitals and came back the same day to work on the scene.
