Employee injured in fall from ladder on top of scaffold

At about 8:50 a.m. on November 4, 1993, Employee #1 was working on a 24-foot extension ladder placed atop a 12-foot high scaffold platform. He overturned the scaffold and ladder. Employee #1 was transported to UCLA Medical Center for emergency treatment of fractures, contusions, and internal injuries. Examination of the scaffold indicated that the ground had not been leveled off near the building, unapproved foundation and bases were used, and insufficient bracing, guying, and lock pins were used.
