Investigation Summary
Employee #1 was doing setup work prior to operating a Sheridan power press used for heat sealing vinyl binder covers. He was cleaning and adjusting the press. He was not using mechanical block-out (a steel block was available but not used) nor was the machine de-energized. He reached into the press to clean some vinyl out and it cycled, smashing his left hand. The hand was amputated above the wrist. A Possons restraint device was available and was used when operating, but not for cleaning and adjusting activities.
