Investigation Summary
At approximately 11:00 a.m. on August 16, 1996, Employee #1, of Steven Bochco Productions in Los Angeles, was inside stage #8 moving a metal cart stacked with 15 to 20 sheets of plywood. After walking away, Employee #1 heard the plywood falling from the cart. The steel bar on top of which the plywood had been stacked tipped over, striking and fracturing his left arm.
