Investigation Summary

Investigation Nr: 170735427
Event: 01/23/1995
Employee injured in fall through wet plywood

Employee #1, a construction grip for Paramount Pictures, was directed to assist two other grips to rig a water pump so that it could be lifted from a lower level room to the upper level with a small crane. The lower level was covered with steel grating except for one small area that was covered with plywood. There was water on the floor and the plywood may have also been wet. There were no witnesses and Employee #1 does not remember exactly what happened, but he fell approximately 20 feet through the plywood area to the concrete floor. He was hospitalized with multiple fractures.
