Employee dies after falling with ladder

At approximately 10:25 a.m. on April 21, 1995, Employee #1, a laborer, was removing an old roof from a small single family dwelling. As he neared a ladder to leave the roof, a coworker cautioned him that the ladder was unsteady. As Employee #1 descended, the ladder began to slip sideways. Employee #1 held onto the ladder as it fell. He struck a decorative cinder block wall with his right side and landed on small concrete steps in a seated position. He never lost consciousness. Employee #1 broke his ribs was transported to the hospital, where he subsequently died during surgery to stop internal (hepatic) bleeding.
