Employee injured in fall from ladder

At approximately 2:00 p.m. on February 14, 1995, Employee #1, of Beachwood Services, Inc., was on an A-frame type ladder about 10 ft above a wood floor at a movie studio, securing a light to a bar clamp. He had been told that the bar clamp had been secured by a grip, but it had not been secured. Employee #1 grasped the bar clamp, which then broke away from its support. He fell to the floor, fracturing his right knee and lower leg.
