Investigation Summary

At approximately 7:15 a.m. on February 8, 1995, Employee #1, of J.D. Goss Construction, used a fixed ladder with steel rungs to descend from the ground level to a basement work station. His rubber soled work boots and the ladder rungs were wet from drizzling clouds. At about the second rung down, his foot slipped off the rung, and he lost hold of the ladder. Falling an estimated 10 or 15 feet, Employee #1 landed on a concrete floor and sustained a compound fracture of his left leg just above the knee. Paramedics transported him to a hospital, where an approximately 18 in. incision was made to install metal pins and screws to immobilize the fracture.
