Employee chips vertebrae in fall from ladder

Employee #1 is a student/worker of the Church of Scientology. Employee #1 was cleaning the awning located at the back of the main building using a section of an extension ladder. She was reaching out to her right when the ladder, which was not secured, flipped and caused her to fall about 8 to 10 feet to the floor. Employee #1 sustained chipped vertebrae. The employer was cited for a serious violation.
