Investigation Summary

Investigation Nr: 120028147
Event: 09/19/1995
Employee's foot partially amputated when run over by bus

At approximately 9:00 a.m. on September 19, 1995, Employee #1, a bus driver for the Golden Gate Bridge, Highway and Transportation District, was in a bus parking lot in San Francisco. He was walking between rows of parked buses when he was struck on the head by the side mirror of a slow-moving bus. The front left tire of the bus then rolled over his right foot. Employee #1's foot was apparently dragged underneath the tire and partially amputated when the bus brakes were applied. He was taken to San Francisco General Hospital, where half of his foot was surgically amputated.
