Investigation Summary
Employee #1, a maintenance man for food-processing equipment, was attempting to quiet a noise in the conveyor of an R-5200 Multivac slicer. He reached into a 1 1/2 in. diameter gear/chain assembly below the conveyor to determine if it was the noise source. He caught the second finger on his right hand in the gear/chain and amputated 1/2 in. of the fingertip. He required hospitalization. Accident-related citations were issued.
