Employee's arm fractured by piping falling in trench

Employee #1 was working on a concrete grouting crew at the bottom of a 55 ft deep shored excavation. They were about to fill 8 to 10 holes that had been drilled to accommodate the grout fill. Piping had been assembled by employees at the top of the excavation (2 1/4 in. OD schedule 80 steel pipe) and secured to a truck-mounted compaction grout pump. Pipe pressures range from 100 psi at start-up to 600 psi when operating. A threaded connection at the top of the excavation blew apart at start-up, the result of inadequate threading. Only four threads of the available 19 at the failed junction were threaded into the joint. Piping and couplings fell 55 to 61 ft into the excavation, striking Employee #1 on his left arm and shoulder. He was hospitalized with a compound fracture of the left arm. An accident-related citation was issued.
