Investigation Summary
At approximately 8:50 a.m. on April 4, 1994, Employee #1 was attempting to make adjustments on the billroom paste conveyor machine's trolley and belt area, which was not guarded. The machine was running when the employee stuck his hand underneath the paste conveyor machine and caught his left hand and glove between the conveyor belt and trolley. Employee #1 sustained a compound fracture and dislocation of the left wrist, for which he was hospitalized. The billroom foreman had also placed his hand in the unguarded portion of the conveyor belt and trolley while it was rotating.
