Employee injures knee in fall from plank to roof

At approximately 9:00 a.m. onFebruary 7, 1994, Employee #1 was walking on a single 2 inch by 12 inch plank to get from a roof area to a catwalk. The employee's right foot slipped off the plank and he fell onto his left knee. He then rolled off the plank to a wider roof area, located directly under the catwalk. The fall from the catwalk to the sloped roof was approximately 4 feet. The roof was at a 4:12 pitch. Employee #1 sustained a torn ligament in his left knee and was transported by the San Francisco Fire Department to Saint Francis Hospital for surgery.
