Investigation Summary

Investigation Nr: 170721997
Event: 12/10/1993
Employee fractures leg in fall from ladder

On December 10, 1993, at approximately 9:30 a.m., Employee #1 was attempting to trim a rafter tail with a hand saw while standing on a fiberglass stepladder without the four legs of the ladder on the floor (the ladder was in the closed position with only two legs on the floor). Employee #1 had reached the second rung of the ladder, when he placed his left foot on a low wall while his right foot was still on the stepladder. His foot slipped off the low wall, he lost his balance, and fell over the side of the building, approximately 22 ft, to the stone sidewalk. Employee #1 sustained a fracture to his left leg (femur) and was transported to San Francisco General Hospital for medical treatment.
