Employee's hip and leg crushed by forklift

Employee #1 works for Circosta Iron and Metal Company. On September 20, 1993, he was emptying scrap metal into a debris box while operating a forklift. Without shutting off the lift, he got off to do something near the debris box. The lift then moved forward toward him. At approximately 10:00 a.m., the forklift pinned and crushed his left hip and leg up against the box. He screamed and a nearby welder came to his aid. The lift was shut off and the paramedics were called. Employee #1 was taken to San Francisco General Hospital and later transferred to Ralph K. Davies Hospital.
