Investigation Summary
An elevator installer and his helper were modernizing elevators. The installer was using a metal fish tape. The fish tape apparently contacted an energized part of a 480-volt circuit and caused an electrical fault. The two employees were burned as a result of the ensuing electric arc. The installer sustained burns on his face and chest. He was hospitalized for his injuries. The helper sustained burns on his hands. He was treated at a hospital and released.
