Investigation Summary
An electrician was instructed by his supervisor to replace a 100-ampere fuse with a 60-ampere fuse, despite the employee's objection. The cover to the fuse box was stuck and could not be opened even had it been deenergized (which it was not). To remove the cover, the electrician pried it with a screwdriver. In doing so, he contacted the energized 100-ampere fuse and caused an electrical fault. (The fault caused all the fuses on the circuit to open.) The employee received an electric shock and severely burned his fingers. He was hospitalized for his injuries. The employer did not have a lockout program, and the employee had not been provided with rubber insulating equipment. The supervisor was not at the jobsite to supervise the injured employee, who was working alone at the time of the accident.
