Investigation Summary

Investigation Nr: 170593479
Event: 11/10/1993
Employee's finger tips were amputated

At approximately 10:00 a.m., on November 10, 1993, Employee #1, a 27 year old laborer, was seriously injured while moving a large trash bin. Employee #1 was guiding the bin from the front side and had his left hand on the corner of the bin while a coworker, the driver of the truck, was pushing it from the rear. The tips of the ring, middle, and index fingers were amputated when Employee #1's left hand was caught between the west door casing and the trash bin being moved through the doorway. The coworker of the two-man crew contacted his company's dispatcher for assistance using his truck radio. A restaurant worker arrived shortly after the accident and reported the accident to his manager, who also summoned emergency medical services. The injured worker's left hand was wrapped in a t-shirt by the coworker. Paramedics arrived and transported the injured worker to a local hospital. Employee #1 returned to light-duty work about 30 days later.
